Prescriptions & Refills
For prescription refills please contact your pharmacy first, they will send the request. Please allow 48 hours notice for routine refill requests. Please be aware that most requests for pain medications and antibiotics require an appointment. Yearly visits are required for stable medication problems, but more frequent visits may bedetermined by your doctor.
Please call 801.479.7787 for prescription refills during regular office hours. No refills will be given after hours, weekends or holidays.
