Your Medical Records
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
Prescriptions & Refills
Medication refills can be requested through your pharmacy. They will then fax us a request. We require 48 hours notice for routine refill requests. Please be aware that most requests for pain medications and antibiotics require an appointment. We require at least yearly visits for stable medication problems, but more frequent visits may be required as determined by your doctor.
Please call for prescription refills during regular office hours. No refills will be given after hours, weekends or holidays.
